·        Please call the school between 8:30-2:30, Monday through Friday at 718-447-2668

 

·        For students entering Grade 1 through Grade 6 in September, we require the latest Progress Report/Report Card and Standardized Tests with your application fee.

 

·        The following is required for any child registering:

 

-          Birth Certificate

-          Immunization Record

-          Baptismal Certificate

-          First Communion Certificate

-          First Penance Certificate

-          IEP Results and Program (if applicable)

-          Report Cards

-          Standardized Tests

 

A $95.00 APPLICATION FEE (CASH OR CHECK MADE OUT TO ST. JOHN VILLA ACADEMY) IS DUE THE DAY OF REGISTRATION.
